Route the chart
Finish your note, then send the chart for cosign and choose the reviewing provider. It appears in their queue with the full note, photos, and products.
Reviewer sign-off
The reviewer opens the chart, confirms the details, and signs. The cosignature is recorded on the record with who signed and when.
Common questions
Can the same person chart and cosign?
That depends on your clinic's supervision rules. Cosign is designed for a second provider to review, which is what most oversight requirements expect.
What if the reviewer requests changes?
They can return it; you update the note and re-route. The history of who did what is preserved.
